Jerome Home, managed by Hartford HealthCare, provides a full service continuum to meet your needs at all stages of life, as well as short-term rehabilitation and out-patient services for people after hospitalization, to support the transition from hospital to home. Jerome Home is a not for profit member of Hartford HealthCare's Senior Health Services.
